If you would like to record the details of a safeguarding incident or concern then please fill in the form below. Please note, this form will automatically be sent only to the PWB Designated Safety Officer.
A copy of the report will be kept by our Designated Safety Office for our records and in case it is needed in further proceedings.

The details of what is recorded is in line with NSPCC guidelines. Please keep the contents of the report and details about the safeguarding incident confidential unless being asked to give evidence by the appropriate authority dealing with the case.
